Community Field Health Officer

Key Responsibilities

  • Health Program Implementation
  • Coordinate and support outreach and health promotion activities in collaboration with community health Promoters (CHPs) and local stakeholders.
  • Support implementation of maternal and reproductive health, Family Planning, child health, nutrition, immunization, and disease surveillance interventions.
  • Identify and coordinate appropriate community responses.
  • Community Engagement
  • Strengthen relationships with community leaders, Unit health committee members, CHPs, and health facility staff.
  • Organize community health education sessions and focus group discussions.
  • Facilitate referrals to health facilities and follow up on service uptake.
  • Data Collection and Management
  • Collect accurate and timely health-related data from communities using mobile tools or paper-based forms.
  • Conduct regular data verification and quality control in collaboration with the M&E team.
  • Maintain organized records and databases of health indicators and field activities.
  • Capacity Building
  • Provide training and mentoring to CHPs on data tools, reporting, and basic health topics.
  • Support health facilities in strengthening community-based data systems.

Qualifications and Experience

Education:

  • A degree or diploma in Public Health, Community Health, Nursing, or a related health field.
  • Additional certification in Data Management, Health Information Systems, or M&E is an asset.

Experience:

  • At least 1–3 years of experience in community-based health programming.
  • Demonstrated experience in collecting and analyzing health data in field settings.
  • Experience working with DHIS2, ODK, Kobo Toolbox, CommCare, or other digital data
  • collection tools preferred.
